The practical way to do it would be to have a HBG with a physical head on his shoulders and one in the box. Using scrim around both heads, you'd fade one light out while the other fades in. The scrim would hide the unlit head.
With your projection, I think perhaps you'd get a better effect if you were able to fade one head in while the other fades out, rather than the swipe effect. It's a neat idea though.
