Thread: coroner form for invitation

I am planning a surprise birthday for my mother, who will be 60 years old.. I picked the toe tags for rsvp and location, but I need a coroner form so I can put in the additional details like potlucks and costumes are encouraged. i tried to search for blank coroner paper everywhere on the internet and cannot find one so far..

do you know where i can find and download the blank form? or did you guys make up one and can let me get a copy?
