Funny gag there. I may just terrorize the neighbors that keep calling the cops on me every year with that one.
But, more practically...
Older kids could double or triple their haul with a half-way well done "little sibling" dummy. You could have it on the arm so it looks like you're carrying him/her around too. Done right, they could even manipulate the masked head to have it seem scared...

A great prank based on a pretty simple idea. Love it.
Made me think of two things.
As per the above, a sturdier TOTer could affix two lightweight dummies on each side of him - say, with a harness or something - each of which would have its own bag. To make it easier to get around, build them a few inches off the ground and then just crouch a bit after ringing the doorbell. Three treats for each visit.
Nobody could possibly be mad at that and would probably have a good laugh at the enterprising young person's ingenuity. If not, run...run like the wind.
What I initially though of, however, was something along these lines. I currently have a robed skeleton sitting by the front door. The head is attached to a white "spine" that is affixed to an old electric garage door opener. When the TOTers ring the bell, whoever is on candy duty pushes a switch and the skelly's head turns in circles, Exorcist-style. It almost always gets a great reaction, whether a jump or a scream.
I've had it for several years now and it is becoming increasingly familiar; maybe it's time for an update. My 12 year-old son was next to me watching the video. What we thought of doing was building a cute little princess dummy, complete with bag, etc... It could be standing by the door, as if waiting for us to answer. When the real TOTers show up on the balcony, a switch is pushed and the cute little princess' face turns towards them and it is something totally unpleasant, perhaps with a blood-curdling scream to boot.
An idea we'll be tinkering with for next year.
