Asylum. I actually worked a mental institute for a few years...

Ok, BEST asylum room I ever saw. Just a guy in harness. It was chained to the ceiling. The line for people to walk by was on the outer wall, this guy would sit in the corner, people enterred, he'ld look, growl, and run, REALLY RUN full bore at you. Harness would catch, he'ld fly up in the air and back to the corner.

Person mumbling incoherently.

Person who hung themselves (covered already) Gotta have it still kicking. It's possible to have it be a real person with a hidden body harness, even.

It's parralell could be a person who slit their wrists.

Person tossing things at the cell door/bars (covered already, very effective)

Consider replacing the electric chair with an electro shock treatment table.

Person caring for a 'baby'. Typically this person would have a doll. I'm thinking a severed head or something would work well for halloween.

Person itching themselves to bleeding. Swearing bugs are inside. (Research morgellon's disease. http://en.wikipedia.org/wiki/Morgellons)

For your children's room, finger painting in blood. Preferably a nurse's blood.

Surgical ward. Labodamy, anyone? HUGE power drill. It might be possible to get/make a dummy bit that looks real but wont' actually drill into anything so the drill could be used with the screaming patient. Hidden pump could even make the drill flip fake blood everywhere.

A death tunnel. For the patients that don't respond to treatment. Conveyor belt full of corpses or something. You could even have all your pallets at the end stacked up, a bunch of body bags on them.
